We get started by designing a care plan that’s customized for each client’s specific needs, including scheduling, daily tasks, and home safety. We will create and share an individualized care plan that is tailored to your loved ones needs.

Finding the Best Fit

We carefully select a caregiver based on similar interests and skills to best match the client’s home health care needs. Our caregivers are trained both annually and one-on-one by our management team to ensure optimal execution of tasks for your loved one’s specific physical and emotional needs.

Getting to Know Each Other

From the beginning, your loved one’s caregiver will study and learn the plan of care prepared at the initial client evaluation. The caregiver will report accomplished tasks and the clinical lead will check to ensure assignments are satisfactorily completed.

Our management team will be there to ensure both the client and guardian/family members are made comfortable with all aspects of the home health care being provided to your loved one.

Checking In and Updates

Our clincical lead monitors the client’s progress based on the caregiver’s daily reports and periodic in-person visits. Family members are kept in the loop via text or phone calls from the clinical lead as new needs or changes arise. Family members also have digital access to our client management system for more detailed insights, so you’ll always know how your loved one is doing.
